As a Clinic Manager at Fresenius Medical Care, you’ll manage the daily operations of our outpatient centers. More importantly, you’ll be the key driver in carrying out our mission: To deliver superior care that improves the quality of life of every patient, every day, setting the standard by which others in the health care industry are judged. This is accomplished by hiring, developing and inspiring a multi-disciplinary team, collaborating with physicians, tracking and delivering superior patient outcomes and monitoring regulatory and safety requirements. Strong management skills are required and prior direct patient care experience is needed for occasions when the team needs a helping hand.

Patient Care Technicians (PCTs) at Fresenius Medical Care support and collaborate with RNs to provide dialysis treatment to several patients at a time while maintaining the functionality of the clinic. This involves prepping and testing equipment, reviewing treatment sheets, and opening the clinic at 4am for first shift. PCTs have a combination of technical skills and genuine passion needed to develop deep relationships with patients and improve their quality of life. Qualified candidates have prior customer service, healthcare and direct patient care experience, and will have the opportunity to learn the discipline of dialysis.

Fresenius Medical Care nurses are on the front line of patient care, improving the quality of life of every patient, every day. That means providing dialysis care to 10-12 patients at any given time, and collaborating with our multi-disciplinary team to deliver a unique care plan to each one of them. This role is all about relationships and the bond you’ll create with patients and their families. Prior dialysis nursing experience is helpful, but many successful candidates come from a background in ICU, PCU and/or surgery.

Inpatient RNs at Fresenius Medical Care perform dialysis treatments on an acute patient population in a hospital setting, including ICU, ER, Rehab, and Med/Surg units. What makes this role truly unique is the level of autonomy you’ll enjoy. Inpatient nurses are specifically accountable for administering and assessing each patient’s treatment plan. And because dialysis is a critical treatment for many hospital procedures and health conditions, you’ll have the opportunity to work with a wide variety of patients. Ideal candidates possess leadership skills and independence needed to make quick decisions in a fast-paced environment. Must be capable of educating patients and providing exceptional customer service to our partners. Six months to one year of dialysis experience or some form of critical care nursing experience are also required.

Job Description

PURPOSE AND SCOPE:

Associate will assist registered nurses with scheduling and care coordination for patients in the Transplant Care Navigation program (TCN). Duties may include but are not limited to supporting patients in a variety of care coordination activities. Responsibilities involve services provided telephonically, email and texting. Charting, coordination and scheduling of patient appointments and arrangement/coordination of patient transportation. Confirm patient intent and attendance of appointments. Reschedule any missed appointment. Assist registered nurses in obtaining test results. Confirm insurance authorization where appropriate.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Under supervision, utilizes established procedures to perform routine assigned tasks.
  • Completes specific competency skills checklist required for job roles. May vary from transplant center to transplant center.
  • Utilizes critical thinking, time management and detailed knowledge of the Care Navigation Program to facilitate the coordination of patient scheduling.
  • Communicates and coordinates with patient and care partners to ensure scheduling attendance.
  • Communicate any potential issues to supervising Registered Nurse and/or management staff.
  • Documents all activities completed and updates charts within the patients' medical record(s).
  • Maintains current knowledge of the departmental program software and the Company's clinical information systems.
  • Assists with various projects assigned by direct supervisor.
  • Learning to use professional concepts. Apply company policies and procedures to resolve routine issues.
  • Works on problems of limited scope. Follows standard practices and procedures in analyzing situations or data from which answers can be readily obtained.
  • Build stable working relationships internally.
  • Normally receive specific, detailed instructions on all work.
  • If applicable, refer to Lead Scheduler or TCN for assistance with day-to-day problems that may arise.
  • Escalates issues to supervisor/manager for resolution, as deemed necessary.
  • Performs other related duties as assigned.
  • Participation in and documentation of training as needed for each Transplant Center

  • This is a work from home role.
  • Day-to-day work includes desk and personal computer work and interaction with patients, facility staff and physicians.
  • The position requires travel between assigned Centers for training only. Travel to Organizational Meetings may be required.

EDUCATION AND REQUIRED CREDENTIALS:

  • Associate’s degree Required
  • Scheduling training preferred
  • EPIC proficiency preferred

EXPERIENCE AND REQUIRED SKILLS:

  • 1 - 2 years' related experience.
  • At least 1 -year experience in clinical setting preferred.
  • Ability to communicate and collaborate effectively and professionally with staff, providers, vendors, and patients in a telephonic environment.t
  • Prior remote employee experience strongly preferred.
  • Ability to multitask and prioritize in a fast-paced environment.
  • Ability to operate basic office equipment such as telephones and computers.
  • Advanced interpersonal skills and ability to handle sensitive and confidential situations and retain confidentiality.
  • Must be capable of clear and timely written/verbal communication with peers, professional staff, providers, and management.
  • Must have good working knowledge of computer systems with emphasis on Word, Excel, and other programs related to the position.
  • Must have a HIPPA-compliant workspace in the home with non-satellite, high-speed internet of more than 20 Mbp.
