As a Clinic Manager at Fresenius Medical Care, you’ll manage the daily operations of our outpatient centers. More importantly, you’ll be the key driver in carrying out our mission: To deliver superior care that improves the quality of life of every patient, every day, setting the standard by which others in the health care industry are judged. This is accomplished by hiring, developing and inspiring a multi-disciplinary team, collaborating with physicians, tracking and delivering superior patient outcomes and monitoring regulatory and safety requirements. Strong management skills are required and prior direct patient care experience is needed for occasions when the team needs a helping hand.

Patient Care Technicians (PCTs) at Fresenius Medical Care support and collaborate with RNs to provide dialysis treatment to several patients at a time while maintaining the functionality of the clinic. This involves prepping and testing equipment, reviewing treatment sheets, and opening the clinic at 4am for first shift. PCTs have a combination of technical skills and genuine passion needed to develop deep relationships with patients and improve their quality of life. Qualified candidates have prior customer service, healthcare and direct patient care experience, and will have the opportunity to learn the discipline of dialysis.

Fresenius Medical Care nurses are on the front line of patient care, improving the quality of life of every patient, every day. That means providing dialysis care to 10-12 patients at any given time, and collaborating with our multi-disciplinary team to deliver a unique care plan to each one of them. This role is all about relationships and the bond you’ll create with patients and their families. Prior dialysis nursing experience is helpful, but many successful candidates come from a background in ICU, PCU and/or surgery.

Inpatient RNs at Fresenius Medical Care perform dialysis treatments on an acute patient population in a hospital setting, including ICU, ER, Rehab, and Med/Surg units. What makes this role truly unique is the level of autonomy you’ll enjoy. Inpatient nurses are specifically accountable for administering and assessing each patient’s treatment plan. And because dialysis is a critical treatment for many hospital procedures and health conditions, you’ll have the opportunity to work with a wide variety of patients. Ideal candidates possess leadership skills and independence needed to make quick decisions in a fast-paced environment. Must be capable of educating patients and providing exceptional customer service to our partners. Six months to one year of dialysis experience or some form of critical care nursing experience are also required.

Empower, engage and train dialysis patients on peritoneal and home hemodialysis using state of the art training programs and equipment
Primarily works in the clinic setting and conducts initial home visits with another staff member present
Position functions as an onsite Patient Educator and Case Manager for patients with various shifts and work hours
This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. Assesses and manages patients’ response to home dialysis training and treatment therapy by following prescribed predetermined protocols and communicates patient related issues to the physician as needed. As a member of the End Stage Kidney Disease (ESKD) health care team, this position participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations.
Qualifications
- Graduate of an accredited School of Nursing.
- Current Registered Nurse (RN) license in the state of practice
- Minimum of 12 months of RN experience
- Current CPR and BLS certification
- Must meet the practice requirements in the state in which he or she is employed
- Current driver's license, reliable transportation, and vehicle insurance in state of residence.
- Basic computer skills and proficiency in MS Word and Outlook
- Previous case management experience (Preferred)
- Excellent verbal and written communication skills
- Ability to work independently as well as with other members of the Home Therapy team, with little to no supervision, in completing all monthly requirements for each patient.
- Excellent time management and organizational skills to ensure tasks are completed on time to prevent negative outcomes for patients.
- Previous Chronic Hemodialysis, Acute Dialysis, Home Hemodialysis (HHD), or Peritoneal Dialysis (PD) nursing experience (Preferred but not required. Will train qualified applicant)
- Bilingual a plus

Responsibilities
- Position requires frequent travel to patient homes ranging from Jasper to Orange to Baytown and surrounding areas.
- Will be trained in both home modalities: PD and HHD
- All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations.
- Successful completion of all FKC education and training requirements for new employees
- Collaborating with the patient’s Nephrologist to determine treatment plans that best works for each patient
- Educate potential patients on different dialysis modality options to determine if any fit their lifestyle and health status.
- Train patients and care partners in the clinical environment on how to perform dialysis in the home environment to improve patient freedom and outcomes.
- Perform case management on current patients to complete ongoing care such as monthly lab draw, medication administration, treatment adherence, education reinforcement, infection prevention, and more.
- Complete training documentation and monthly/annual routine assessments.
- Rotating on-call after 1 year (phone only) with the help of an answering service to help patients when office is closed
- Work closely with other members of the IDT to identify any issues or barriers find resolutions to ensure best patient outcomes
- Complete routine quality reports and audits.
- Maintain a positive working relationship with other members of the HT staff, IDT, Nephrologist offices, access centers and other professionals within the care of the patient.
- Initial training has requirements for 3 – 5 individual days in Houston for face to face training and skills check offs. These are scheduled in advance and hotel accommodations are available.
- Maintain a valid nursing license, CPR and BLS
- Maintain valid drivers license with reliable transportation
- Complete annual and continuing virtual/on-line education
- The position provides direct patient care that regularly involves heavy lifting, moving of equipment, patients and assisting with ambulation. Equipment aids and/or coworkers may provide assistance as needed.
- The employee may occasionally be required to move, with assistance, machines and equipment of up to 200 lbs., and may lift chemical and water solutions of up to 30 lbs. as high as 5 feet.
- Day to day work includes desk work, computer work, interaction with patients, facility/hospital staff and physicians.
- Schedule: 8am – 4:30pm CST Monday – Friday with options for holidays off depending on clinic needs. May be expected to start earlier or stay late depending on patient training needs.
- Staff may be required to travel and assist with other training needs in Houston area if schedule permits.
- Other tasks as needed and assigned by supervisor
