As a Clinic Manager at Fresenius Medical Care, you’ll manage the daily operations of our outpatient centers. More importantly, you’ll be the key driver in carrying out our mission: To deliver superior care that improves the quality of life of every patient, every day, setting the standard by which others in the health care industry are judged. This is accomplished by hiring, developing and inspiring a multi-disciplinary team, collaborating with physicians, tracking and delivering superior patient outcomes and monitoring regulatory and safety requirements. Strong management skills are required and prior direct patient care experience is needed for occasions when the team needs a helping hand.

Patient Care Technicians (PCTs) at Fresenius Medical Care support and collaborate with RNs to provide dialysis treatment to several patients at a time while maintaining the functionality of the clinic. This involves prepping and testing equipment, reviewing treatment sheets, and opening the clinic at 4am for first shift. PCTs have a combination of technical skills and genuine passion needed to develop deep relationships with patients and improve their quality of life. Qualified candidates have prior customer service, healthcare and direct patient care experience, and will have the opportunity to learn the discipline of dialysis.

Fresenius Medical Care nurses are on the front line of patient care, improving the quality of life of every patient, every day. That means providing dialysis care to 10-12 patients at any given time, and collaborating with our multi-disciplinary team to deliver a unique care plan to each one of them. This role is all about relationships and the bond you’ll create with patients and their families. Prior dialysis nursing experience is helpful, but many successful candidates come from a background in ICU, PCU and/or surgery.

Inpatient RNs at Fresenius Medical Care perform dialysis treatments on an acute patient population in a hospital setting, including ICU, ER, Rehab, and Med/Surg units. What makes this role truly unique is the level of autonomy you’ll enjoy. Inpatient nurses are specifically accountable for administering and assessing each patient’s treatment plan. And because dialysis is a critical treatment for many hospital procedures and health conditions, you’ll have the opportunity to work with a wide variety of patients. Ideal candidates possess leadership skills and independence needed to make quick decisions in a fast-paced environment. Must be capable of educating patients and providing exceptional customer service to our partners. Six months to one year of dialysis experience or some form of critical care nursing experience are also required.

Job Description

Purpose and Scope

The Nephrology Nurse Practitioner (NP) provides advanced practice medical care to patients with acute and chronic kidney disease across dialysis, outpatient clinic, and inpatient hospital settings. The NP functions as a licensed independent provider, delivering comprehensive evaluation, diagnosis, treatment, and disease management, either independently or in collaboration with a supervising or consulting nephrologist, in accordance with state scope-of-practice regulations.

The NP is accountable for clinical decision-making, longitudinal disease management, and care coordination to optimize patient outcomes, quality, and safety.

Principal Duties and Responsibilities

Direct Patient Care – Provider Level

  • Perform comprehensive histories, physical examinations, and clinical assessments for nephrology patients in dialysis units, outpatient clinics, and hospital settings.
  • Diagnose and manage acute and chronic kidney conditions, including but not limited to chronic kidney disease (CKD), end-stage kidney disease (ESKD), hypertension, electrolyte disorders, anemia, mineral bone disease, and volume management.
  • Independently develop, implement, and modify evidence-based treatment plans, including prescribing medications, ordering diagnostics, and adjusting dialysis parameters as clinically indicated.
  • Conduct routine and problem-focused dialysis patient rounds, addressing adequacy, access function, anemia management, blood pressure control, and complications of dialysis therapy.
  • Manage transitions of care, including hospital admissions, discharges, and follow-up coordination, to ensure continuity across care settings.

Procedures and Clinical Oversight

  • Perform and/or manage nephrology-related procedures within the scope of practice and credentialing (e.g., catheter assessments, vascular access evaluations, fluid management, medication titration).
  • Interpret laboratory data, diagnostic studies, and imaging results to guide medical decision-making.
  • Identify and manage urgent and emergent renal conditions, escalating care or consulting physicians as clinically necessary.

Collaboration and Care Coordination

  • Collaborate with nephrologists, dialysis staff, hospital teams, and interdisciplinary providers to coordinate complex patient care plans.
  • Serve as a clinical resource and consultant to nursing and dialysis staff regarding patient management and care protocols.
  • Participate in multidisciplinary rounds and care conferences.

Patient Education and Counseling

  • Provide comprehensive education to patients and families regarding kidney disease progression, dialysis modalities, medications, nutrition, lifestyle modification, and advance care planning.
  • Support shared decision-making related to dialysis initiation, modality selection, and conservative care options.

Documentation and Compliance

  • Accurately document all clinical encounters, assessments, diagnoses, treatment plans, and outcomes within the electronic medical record in compliance with regulatory, billing, and quality standards.
  • Ensure compliance with federal, state, and organizational policies, clinical practice guidelines, and standards of care.

Quality, Leadership, and Professional Practice

  • Contribute to quality improvement initiatives, outcomes monitoring, and clinical performance measures within nephrology services.
  • Mentor and support nursing and clinical staff through clinical leadership and knowledge sharing.
  • Maintain current clinical expertise through continuing education and professional development in nephrology and advanced practice.

Education and Licensure

  • Master’s degree or higher from an accredited Nurse Practitioner program.
  • Current Nurse Practitioner license with prescriptive authority in applicable state(s).
  • National board certification as a Nurse Practitioner (adult, family, or acute care as appropriate).
  • CPR/BLS certification (ACLS as required by practice setting).

Experience and Skills

  • Minimum of 1–3 years of Nurse Practitioner experience preferred; nephrology experience strongly preferred.
  • Demonstrated ability to practice independently, manage complex chronic disease, and make advanced clinical decisions.
  • Strong clinical judgment, communication, and interdisciplinary collaboration skills.
  • Proficiency with electronic medical records and clinical documentation systems.
