Descrição:
Atuar como ponto de referência de acolhimento dos pacientes, especialmente na admissão e nos primeiros três meses de tratamento.
Articular e identificar as informações, sendo responsável pela análise da realidade social dos pacientes, estabelecendo um diálogo entre estes, familiares e equipe, tendo como principal objetivo, responder às demandas destes pacientes, garantindo, e se possível, ampliando o acesso aos direitos assegurados na constituição federal e na legislação complementar, buscando a melhoria das condições de vida do paciente renal e proporcionando Boas Experiências no ambiente da clínica.

Ensino Superior completo em Serviço Social;
Experiência prévia com atendimento clínico em Nefrologia;
Desejável Pós graduação;
Disponibilidade para cumprir 30 horas semanais
